Our team of movement specialists knows that each individual is unique and requires a plan that is tailored to their needs. The foundation of our assessment is grounded in the principles of Applied Functional Science® (AFS), and we address injuries and dysfunction both locally and at the source of breakdown. Arizona is a Direct Access state, meaning you can come to Spooner for any musculoskeletal concern without a referral or a prescription from your physician. We hope to be your first call for any and all of your musculoskeletal needs, aches, or pains.
While we are excited to help everyone, your insurance provider may still require a referral in order to subsidize your visit. We recommend familiarizing yourself with your health insurance benefits by calling your insurance provider. If you plan to pay out of pocket, there are no barriers in place to prevent you from receiving the care you need.

Wear comfortable, loose-fitting clothing that allows for ease of movement. Basketball or running shorts, leggings, tee shirts, and sneakers are all acceptable. If you have an injury to a lower extremity that will require examination and/or manual therapy, please bring or wear shorts to your visit.
On your first visit, you’ll need to bring a photo ID, your insurance card, a prescription from your doctor (if applicable). These documents will be used to verify your identity and ensure a smooth billing process. You will not need these documents during your future visits.
Your first scheduled appointment will start with an evaluation by your therapist. Your therapist will conduct an interview, medical history review, and discuss your goals. Next, your therapist will complete a thorough assessment. This assessment may include functional movement or an analysis of your body’s entire kinetic chain.
Together, you and your therapist will develop a treatment plan based on your functional movement goals. At the end of the first visit, your therapist will provide you with a home exercises program and help plan out any future appointments that might be necessary.
During each appointment, you’ll meet with your therapist to review the outcome of your previous appointment, discuss how you’ve been doing since your last appointment and review your successes with your home exercise program. You’ll review current pain levels, stressors and anything else relevant to your plan of care.
Your therapist may perform hands-on and/or instrument-assisted therapies to help improve joint and tissue function. You will also perform specific exercises during your appointment that are designed to teach your body how to move and function in healthy ways.
Your therapist will review your progress during each visit by measuring your mobility, strength, range of motion, pain levels, successes, or ability to complete functional tasks. Physical measurements are taken to record improved mobility, muscle regeneration and growth. Your therapist will assess not only your physical progress but also your emotional wellbeing as you progress through treatment. Ensuring our patients are succeeding in mind, body, and spirit is a commitment we make to every patient.
These observations will help the therapist in progressing or modifying your treatment as you continue your journey through physical therapy.
Please plan to arrive to your initial evaluation 15 minutes prior to your scheduled appointment time. This will allow us an opportunity to review your new patient paperwork and ensure your questions are answered.
Please plan to arrive 5-10 minutes prior to any follow-up appointments.
Recovery time is unique to you and does vary on a case-by-case basis. Some patients can expect 6-8 weeks of therapy. Cases that are more complex will likely remain in physical therapy longer.
If you are going to be late or unable to make an appointment, please call the clinic at your earliest convenience as a courtesy to your therapist. We request a 24 hour notice for cancellations, so we are able to adjust the schedule and provide an opportunity for other patients to see your therapist.
Missed appointments or failures to notify the clinic at least 24 hours in advance of your appointment may be subject to a $25 cancellation fee.
We understand that life and professional/personal responsibilities can cause complications in your schedule. If you are running late to an appointment, please communicate with your therapist and front office to ensure open dialogue. If possible, our team will gladly make the adjustments necessary to ensure uninterrupted care.

In most cases, your insurance plan has some level of physical therapy benefit to cover or offset the out-of-pocket costs associated with your therapy visits. With insurance, you are responsible for any out-of-pocket expenses related to your appointment. These expenses will vary and are determined by your health insurance plan. These typically include any co-pay, deductible, and/or co-insurance.
Because transparency is important to us, we make every effort to verify benefits and communicate your responsibility to you prior to your initial evaluation. We also encourage you to check your plan details, know your payment responsibilities as outlined in your plan, and communicate your needs to your insurance provider.
Medicare patients: Physical therapy is covered by your Medicare plan. Your physical therapist or hand therapist will work with you to develop a Plan of Care that meets Medicare requirements. If you have questions about your Medicare benefits, please feel free to discuss with your clinics front-office team or your therapist.
Workers’ Compensation patients: Your medical expenses are covered under Arizona Workers’ Compensation Law.
Please discuss cash pricing with your Spooner therapist or front-office. If you would like to pre-purchase visits, we do offer a “Health in Motion Package” designed to reduce your out-of-pocket cost by bundling appointments. Ask the front-office at your Spooner clinic for more details about these discounted plans or email billing@spoonerpt.com with questions.
At your Spooner clinic, we are able to accept credit cards, debit cards, and mobile payments.
We request that payments made in cash or check be mailed to our billing team along with your statement.

Payment for patient responsibility (co-pay, deductible, and/or co-insurance) is due at the time or care.

There are several options available to you for making a payment:

Payment plans and scheduled payments can be set up by calling the Patient Services Line at 480-551-4977. All payments will be tracked by the Spooner team and will conclude once the outstanding balance is paid in full.
Movement Plus membership payments should be handled through the Movement Plus web portal.
