A MESSAGE FROM SPOONER
Dear Athlete,
Injuries can happen when you least expect them. From training, to practice, to your game, you are not alone when it comes to the assessment and treatment of your musculoskeletal needs.
Spooner will provide you with expert care as we are committed to helping you achieve your best athletic performance. By granting Spooner permission to treat your injury, we will assess, spot treat, educate, and provide injury prevention strategies. It is our priority to communicate with you clearly and directly during your course of treatment.

AUTHORIZATION TO ASSESS AND TREAT
All procedures will be thoroughly explained to the athlete before they are performed. There are certain inherent risks with injury screens, movement assessments and treatment. These could increase the current level of pain or discomfort or could aggravate an existing injury. There is also a possibility of experiencing a new injury, but the risk is small. The therapist/trainer will take every precaution to ensure the athlete is protected, and the athlete will be able to stop the assessment and/or treatment at any time. Based on this information, I understand and authorize Spooner to perform appropriate assessment and/or treatment techniques based on a general assessment of my or my child’s condition.
MEDIA RELEASE
I grant permission to Spooner and its agents and employees the irrevocable and unrestricted right to reproduce the photographs and/or video images taken of me for the purpose of publication, promotion, illustration, advertising, or trade in any manner or in any medium. I hereby release Spooner and its legal representatives for all claims and liability relating to said images or video. I waive my right to any compensation.
